Job summary

Pinnacle Services, Inc. is seeking a Case Manager for the Hennepin County Float. This full-time role works with individuals on CADI, DD, BI or AC/EW waivers who may have various disabilities or age-related needs.

You will assess needs, coordinate resources, develop service plans, and attend interdisciplinary team meetings. The position involves documenting work for billable units, managing caseloads (10–40), and thriving in high-volume periods with increased communications.

Qualifications

• Bachelor’s degree in social work or related field.
• Valid driver’s license and acceptable driving record.
• Successful local/background checks as part of hiring process.

Responsibilities

• Assess areas of persons’ lives where assistance is needed.
• Offer and coordinate resources for the persons served.
• Monitor service delivery and work with providers to ensure needs are met.
• Develop service plans.
• Attend regular in-person meetings with clients and interdisciplinary teams.
• Document client-related work to create billable units.
• Manage 10-40 cases at a time; keep the float position under 50 clients.
• Adapt to high-volume periods, such as large case transfers with increased emails and calls.

Education

Bachelor's degree in social work or related field

Job Responsibilities and Duties

The Case Manager Hennepin County Float position works with persons served on CADI, DD, BI and/or AC/EW waivers. These individuals may have developmental disabilities, serious and persistent mental illness, chemical dependency issues, physical disabilities or may be aging.

• Assess the areas of persons’ lives where assistance is needed.


• Offer and coordinate resources for the persons served.


• Monitor service delivery and work with providers to ensure needs are met.


• Develop service plans.


• Attend regular in‑person meetings with clients and interdisciplinary teams.


• Document client‑related work to create billable units.

Salary

Non-LSW: $51,000 per year (40 hrs/week). LSW: $51,600 per year (40 hrs/week).

Required Qualifications

• Bachelor’s degree in social work, psychology, sociology, or closely related field.


• Alternative: Bachelor’s degree in any field with one year of full‑time experience as a social worker/case manager/care coordinator in a public or private social service agency.


• At least one course focused on developmental disabilities, verified by transcript.


• Valid driver’s license and acceptable driving record.


• Vehicle with current insurance.


• Successful local/background checks.

Preferred Qualifications

• At least one year of full‑time experience in education or treatment of persons served with developmental disabilities or related conditions.


• LSW or higher form of BOSW licensure.
