Job summary
CAMBA is seeking a full-time Case Manager for the Citywide Scattered Site Housing Program in New York, NY. You will provide comprehensive case management services to chronically homeless individuals with serious mental illness to help them maintain stable housing. Responsibilities include conducting assessments, connecting clients to services, monitoring progress, and completing all necessary documentation. A Bachelor's degree or equivalent experience in social services is required, as well as computer proficiency and bilingual skills. Compensation is $51,500 annually with comprehensive benefits.
Qualifications
Bachelor or Associate degree with one year of equivalent experience.
Two years of equivalent experience with sufficient education and technical expertise.
Relevant experience preferably in organizations working with at-risk populations.
Responsibilities
Conducts assessments, service plans, and advocacy for clients.
Connect clients to medical, mental health, and legal services.
Monitor and report on clients' progress and living conditions.
Skills
Bilingual English and Spanish
Computer literacy in Microsoft Office Suite
Experience with clients having histories of homelessness, mental illness, or substance abuse
Education
Bachelor or Associate degree
