Premera is hiring Case Managers (RN, RD, Licensed MH Clinicians) to partner with members, providers and caregivers within an integrated care model. Roles focus on engagement, assessments and collaborative care planning aimed at improving health outcomes and patient self-management.

You will utilize evidence-based skills to promote behavior change, ensure care coordination, and provide post-discharge follow-up while driving quality outcomes. Training and a supportive environment are provided.

Competenze

• Active licenses required: RN, RD, or licensed mental health clinician.
• Experience in integrated case management or care coordination.
• Strong engagement, telephonic and digital communication skills.
