Job summary

Cambia Health Solutions, Inc. is seeking a Care Management Clinician to provide case, disease, and care coordination services. The role supports member health needs with independent clinical assessment and collaboration with physicians to ensure quality and cost-effective outcomes.

The position offers flexible work-from-home options across multiple western states and pays on an hourly basis with a target bonus. Candidates should have a nursing degree and active licensure.

Qualifications

• Associate or Bachelor’s Degree in Nursing and 3 years in case management or related field.
• Licensure or certification allowing independent assessment in their discipline and at least 3 years of direct clinical care.
• URAC case manager cert, or health-related degree, or RN license with scope for medical care management.

Responsibilities

• Perform end-to-end case management activities: assessment, planning, implementation, coordination and evaluation.
• Apply clinical judgment to ensure medical policy compliance and appropriate care standards.
• Consult with physician advisors to determine clinically appropriate decisions.
• Collaborate with departments to resolve claims, quality of care, and provider issues.
• Document plans and actions clearly to meet timelines and performance goals.
