Job summary

Qualifications

• Minimum 2 years of acute care hospital patient care experience required.
• Preferred 2 years of experience as acute care case manager.
• Current unrestricted RN license in the State of Nevada.
• Ability to coordinate diverse groups in a fast-paced environment and strong problem-solving skills.

Responsibilities

• Promotes current National Standards for case management.
• Transition management: discharge planning and readmission prevention.
• Utilization management: assess medical necessity and resource use.
• Coordinate care with interdisciplinary team and ensure efficient throughput.
• Document activity according to policy and support post-acute care education.
