Job summary

Qualifications

• Bachelor’s degree in a health-related field is required.
• Licensure as a social worker, RN, or counselor is preferred and must be maintained.

Responsibilities

• Engage clients to assess needs and strengths and gather data for care continuity.
• Develop individualized, client-centered treatment plans with clients.
• Assist clients in setting measurable goals and facilitating plan implementation.
• Review and update plans at 3-month intervals and as needs change.
• Monitor service delivery and adherence to the treatment plan.
• Document plans and case management activities for clinical and billing purposes.
• Maintain electronic case files and uphold productivity standards.
