AMG Specialty Hospital- Central Indiana Hancock Campus in Greenfield, IN seeks a Full-Time Case Manager (RN or LPN) to coordinate discharge planning and post-discharge services for patients and families. The role requires Indiana RN or LPN license, current BLS and ALS, and strong English communication.

Responsibilities include admission/discharge coordination, case conferences, insurance relations, and reporting within managed care systems, with a focus on patient rights and quality outcomes.

Qualifications

• 6 months to 1 year discharge planning experience preferred.

• 1–3 years of related case management experience.

The Case Manager's primary function isto make readily available to the patient, their families and/or significantothers, special services in order to facilitate adjustment of theseindividual's continuity of care post discharge. The Case Manager overseesthe effective coordination of admission and discharges planning, casecommunications, conferencing, insurance relations and assurance through managedcare systems, and verbal and written reporting. Assures regulationsregarding patient rights are fulfilled. All candidates must have a RegisteredNurse (RN) Indiana License in good standing.

Job Requirements

• Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) Indiana License in good standing.
• Minimum of six months to one year of discharge planning experience preferred.
• If LPN, and no experience as above, supervision by RN will be required during a 6-month training period.
• Current BLS certification.
• Current ALS certification.
• Minimum of 1-3 years of related workexperience as a Case Manager.
• Ability to read andcommunicate effectively in English.
