Job summary

AMG Specialty Hospital- Central Indiana Muncie Campus is seeking a Full Time Case Manager (RN, LPN). The role focuses on coordinating admission and discharge planning, communicating with families, and ensuring continuity of care post-discharge on a busy LTAC unit.

Applicants must hold an Indiana RN or LPN license, current BLS and ALS certifications, and 1–3 years of case management experience. English communication is required.

Qualifications

• Current RN or LPN Indiana license in good standing.
• BLS certification currently active.
• ALS certification currently active.
• 1–3 years of case management experience preferred.
• Ability to read and communicate effectively in English.

Responsibilities

• Coordinate admission and discharge planning and related communications.
• Oversee insurance relations and managed care coordination.
• Provide timely verbal and written case reporting and conferencing.
• Ensure compliance with patient rights regulations.

AMGSpecialtyHospital -Muncie Campus is seeking aFull Time Case Manager (RN, LPN). TheCase Manager's primary function is to make readily available to the patient,their families and/or significant others, special services in order tofacilitate adjustment of these individual's continuity of care postdischarge. The Case Manager oversees the effective coordination ofadmission and discharges planning, case communications, conferencing, insurancerelations and assurance through managed care systems, and verbal and written reporting.Assures regulations regarding patient rights are fulfilled. Allcandidates must have a Registered Nurse (RN) or Licensed Practical Nurse (LPN) Indiana License in good standing.
