Responsibilities

Ensures appropriate utilization of hospital services as well as developing a discharge plan and all appropriate services in a timely manner. Recognizes the signs and symptoms of stroke and calling a stroke alert overhead. Verbalizes stroke alert process. Works with the stroke patient and family for discharge planning and treatment based on needs, condition and prognosis of patient. Maintains currency in stroke knowledge by completing the annual 1 hour online training.
