Job summary

Wellington Regional Medical Center in Wellington, FL, part of the UHS family, is seeking a Per Diem Case Manager. The role focuses on ensuring appropriate utilization of hospital services, developing timely discharge plans, and coordinating post-acute care for stroke and other conditions.

The ideal candidate is a Florida-licensed RN with 2–4 years recent acute care experience, knowledge of InterQual criteria, strong communication skills, and familiarity with discharge planning processes.

Job Requirements:

Ensures appropriate utilization of hospital services as well as developing a discharge plan and all appropriate services in timely manner. Recognizes the signs and symptoms of stroke and calling a stroke alert overhead. Verbalizes stroke alert process. Works with the stroke patient and family for discharge planning and treatment based on needs, condition and prognosis of patient. Maintains currency in stroke knowledge by completing the annual 1 hour online training
