Universal Health Services, Inc. is seeking a Case Manager to work with the treatment team and Director of Social Services to coordinate discharge planning, aftercare, and placement for patients requiring residential or outpatient services.
The role involves advocating for patients, coordinating with community resources and external review organizations, and maintaining timely communication with families and providers to ensure smooth transitions of care.
The Case Manager functions under the direct supervision of the Director of Social Services. He or she assumes responsibility for working with the treatment team and the patient in order to assist in basic discharge planning needs, coordination of aftercare and placement of the patient. This includes residential needs, aftercare needs, advocacy needs, entitlement needs, and building relationships with aftercare providers. Other duties as assigned by supervisor.

Education/Training:
Experience: A minimum of two (2) years direct clinical experience in a psychiatric or mental health setting, with direct experience in case management patient assessment, patient aftercare coordination, and communication with external review organizations. Experience in working with community resources, residential/outpatient programs and outside referrals.
Knowledge and Skills: Extensive knowledge of community resources and FLBHC’s referral sources in order to make appropriate referrals.
