Qualifications

  • Bachelor’s degree in Nursing required.
  • PA RN license required.
  • Complete required competencies and trainings as per role.

Job Summary

The RN Case Manager is a registered nurse responsible for coordinating the care and services for an assigned unit-based patient population. The Case Manager promotes quality of care, supports effective resource utilization, and facilitates the patient's progression of care to proactively plan for the patient's safe and timely discharge to the most appropriate level of care. The Case Manager serves as the key resource for the patient's multi-disciplinary care team and works collaboratively with all members of the patient's health care team and patient representatives to achieve desired clinical, financial and quality outcomes. The Case Manager analyzes current systems and variances to prevent delays and identify opportunities for improvement. This position reports to the Manager, Case Manager or Director. Case Management is collaborative process, which assesses, collaborates, plans, implements, coordinates, monitors, and evaluates options and services to meet an individual's health needs through communications and available resources to promote quality, cost-effective outcomes. All Case Manager activities will be patient centered, proactive, require on-going assessment of needs, evaluation of resources, and education as needed. The Case Manager is responsible for presenting options in an unbiased manner and meeting compliance with HIPPA. Advocacy and education are at the heart of all activities focusing on providing the best possible clinical outcomes. Case Management activates its processes to secure the right service to the right patient at the right time at the right level of care.

Schedule

Schedule: 4 \"10\" hour shifts, with Tuesday off. 7:30a - 6p and 8a - 6:30p hours to be worked.



Education Requirements


  • 4 year Bachelor's Degree - OR - combination of relevant education & experience may be considered in lieu of Bachelor's Degree

  • Completes and maintains all competencies and trainings as required for role.
