Job summary

AMG Specialty Hospital- Central Indiana Hancock Campus in Greenfield, IN seeks a Full-Time Case Manager (RN) to coordinate post-discharge services and support patients and families. The role includes coordinating admissions, discharges, communications, and managed care processes to ensure seamless continuity of care.

The ideal candidate holds an Indiana RN license, BLS and ALS certifications, plus 1–3 years of case management experience, and can read and communicate effectively in English.

Qualifications

• Current Registered Nurse (RN) Indiana License in good standing.
• Current BLS certification.
• Current ALS certification.
• Minimum of 1-3 years of related work experience as a Case Manager.
• Ability to read and communicate effectively in English.

Responsibilities

• Coordinate admission and discharge planning for patients and families.
• Oversee case communications, conferencing, and reporting.
• Manage insurance relations and utilization through managed care systems.
• Ensure compliance with patient rights regulations and documentation.

The Case Manager's primary function isto make readily available to the patient, their families and/or significantothers, special services in order to facilitate adjustment of theseindividual's continuity of care post discharge. The Case Manager overseesthe effective coordination of admission and discharges planning, casecommunications, conferencing, insurance relations and assurance through managedcare systems, and verbal and written reporting. Assures regulationsregarding patient rights are fulfilled. All candidates must have a RegisteredNurse (RN) Indiana License in good standing.
