Job summary

Tampa General Hospital is seeking a Case Manager RN for Care Coordination to ensure safe, timely transitions of care across levels of care. You will collaborate with physicians and the interdisciplinary team to develop, implement, and monitor individualized care plans addressing LOS and discharge needs.

The role emphasizes barrier identification, documentation, and coordination with Social Work to reduce readmissions and meet regulatory requirements.

Qualifications

• Nursing degree or equivalent licensure.
• Five years nursing experience with at least two years in Case Management or two years in Emergency Medicine.

Responsibilities

• Develops, documents, and coordinates Discharge Plan A and Plan B to support safe transitions of care.
• Collaborates with physicians and the interdisciplinary team to determine plan of care, LOS, and discharge disposition.
• Participates in rounds and huddles to communicate patient status and alignment of care progression goals.
• Ensures patients progress through clinical milestones and adjusts discharge dates as clinically indicated.
• Identifies barriers to care progression and escalates issues for resolution.
• Coordinates alternate levels of care when needed.
• Partners with Social Work to arrange post-acute services to reduce readmissions.
• Monitors and documents avoidable days, discharge dates, and utilization findings.
• Engages in performance improvement initiatives and readmission reviews.
