Job summary

Ardent Health Services in Topeka, KS is seeking a variable shift, full-time Case Management Acute Care Case Manager RN to coordinate patient care across the continuum and determine medical necessity using evidence-based guidelines.

The RN will report to the Director of Case Management and join a team focused on reducing unnecessary utilization while achieving optimal clinical outcomes and providing resource management support.

Qualifications

• RN license required.
• Associates degree in nursing or higher.
• 2+ years’ acute hospital-based nursing experience.
• Certification in case management (ACM or CCM) preferred.

Responsibilities
