Job summary

Qualifications

• RN license required.
• Associates degree in nursing or higher.
• 2+ years’ acute hospital-based nursing experience.
• Certification in case management (ACM or CCM) preferred.

Responsibilities

• Assess and coordinate patient care across the continuum, using criteria to manage benefits and determine medical necessity.
• Aim for optimal clinical and quality outcomes by managing care and resources to reduce unnecessary utilization.
• Reports to the Director of Case Management or designee.
