Qualifications

• Certification in case management (ACM or CCM) preferred.

Responsibilities

• Assess and coordinate patient care across the continuum using criteria to manage benefits.
• Manage care and resources to reduce unnecessary utilization.
• Reports to the Director of Case Management or designee.
