Qualifications

• Must have a valid Driver's License and proof of liability insurance.
• Provide/maintain all required immunizations and/or vaccinations.
• Complete all required background checks.

Responsibilities

• Conduct comprehensive assessments on assigned clients.
• Develop a service plan with clients.
• Provide community, school, and home visitation.
• Complete all paperwork associated with maintaining current client statistics.
