Job summary

CCAP in Rhode Island seeks a dedicated case manager to provide care coordination for individuals and families in the RI Works program. You will ensure compliance in key components and help resolve barriers to progress.

Requirements include a bachelor’s degree and at least 2 years of related experience, along with valid driver’s license. Bilingual skills in Spanish or Creole are preferred, with flexible scheduling to serve clients in various settings.

Qualifications

• Bachelor's degree required; preferred.
• Minimum of 2 years related experience.
• Demonstrated case management skills and experience.
• Valid Driver’s License and proof of liability insurance.
• Bilingual in Spanish and/or Creole preferred.

Responsibilities

• Conduct comprehensive assessments including risk, basic needs, and barriers to progress.
• Develop a service plan with clients per program guidelines.
• Provide community and/or home visitation to clients on caseload.
• Instruct on daily living skills relevant to education and employment goals.
• Assist with emergency funding, housing, utility, and benefits applications.
• Maintain accurate client records and data in agency systems.
• Collaborate with CCAP programs and attend relevant meetings and trainings.
