Job summary

Universal Health Services is seeking a Case Manager to coordinate aftercare and connect Compass patients with community and military resources. You will mentor staff, collaborate with clinical leadership, and drive individualized treatment plans to promote well-being and functional independence.

The role emphasizes close collaboration with the multidisciplinary team and a commitment to transitioning clients toward discharge as stability improves.

Qualifications

• Bachelor's degree required; Master's degree preferred.
• Minimum of 2 years of experience in case management related work.
• Completion of Crisis Prevention and Intervention (CPI) training within 30 days of employment

Responsibilities

• Coordinate and facilitate the delivery of quality services to Compass patients.
• Mentor case management staff and collaborate with Clinical Program Directors.
• Coordinate aftercare needs based on individualized treatment plans.
• Utilize community and military resources to support clients' independence.
• Assist with transition toward discharge as stability improves.
