Job summary

Belmont Pines Hospital is seeking a Case Manager to coordinate care for patients and families, acting as a liaison among the treatment team, physicians, referrals, and community resources. The role requires meticulous record-keeping and collaborative planning to support treatment goals.

You will maintain medical records, assist with master treatment planning and discharge planning, and follow up with discharged patients to ensure smooth transitions and timely referrals.

Qualifications

• Bachelor's degree in social services field.
• Experience with children and adolescents.
• Background check to include FBI/BCI and drug screen required.

Responsibilities

• Serve as liaison between treatment team, physician, referral source, community agencies, outside therapists, and third party reviewers.
• Maintain the integrity of the medical record for assigned patients and assist in preparing the master treatment plan.
• Initiate discharge planning and contact discharged patients within established follow-up time frames; assist in completing intake screenings and referrals.
• Perform other related duties as assigned by the Supervisor.
