Job summary

Universal Health Services in Youngstown, OH is seeking a Case Manager to coordinate care for patients and families within the treatment team. You will serve as a liaison among physicians, referral sources, and community agencies, and you will help maintain accurate medical records and assist in discharge planning.

You will initiate follow-up with discharged patients, support intake screenings and referrals, and perform other duties as assigned by the Supervisor.

Qualifications

• Bachelor's degree in social services field.
• Experience with children and adolescents.
• Background check to include FBI/BCI and drug screen required.
