Job summary

Neighborhood Service Organization Inc. is seeking a Case Manager to assess, coordinate, and implement client-centered care plans with the IDT. You will connect team members to maximize services and reduce ER use, while ensuring adherence to treatment and housing goals.

The role requires a bachelor’s degree in a human services field and 2+ years in community-based mental health case management, with hybrid work arrangements available after onboarding.

Qualifications

• Bachelor’s degree in a Human Services field is required.
• Experience in community-based mental health case management is required.
• Familiarity with EMR documentation and Windows-based tools.

Responsibilities

• Provide case management and coordinate care plans to maximize client outcomes.
• Collaborate with IDT to ensure services and benefits are optimized.
• Document assessments and IPOS/case plans in the EMR within policy timeframes.
• Monitor progress toward treatment goals and ensure client satisfaction and HIPAA compliance.
