Job summary

CNS Healthcare is hiring a Case Manager to join our Oakland County Offices. The role focuses on coordinating community-based, integrated health services for individuals with mental health and substance use needs, using trauma-informed care and motivational interviewing to promote wellness.

Qualifications

• QMHP credential required
• Bachelor's degree in a human services field
• Valid driver’s license and transportation access
• Knowledge of mental illness and related treatments
• Proficiency with Microsoft Office and EHRs
• Ability to work collaboratively and meet outcomes

Responsibilities

• Provides community-based case management and integrated services
• Conducts assessments and coordinates care
• Provides transportation assistance
• Facilitates weekly groups
• Completes documentation per CNS policy
• Develops and implements Person-Centered Plans
• Coordinates medication management
• Collaborates with primary and specialty care providers
