Job summary

PAM Health Rehabilitation Hospital of Richardson is seeking a Case Manager to coordinate inpatient care across disciplines and ensure proper utilization and discharge planning.

You will work with the interdisciplinary team to facilitate achievement of financial and quality outcomes, maintain payor relationships, and support high-quality, patient‑centered care in a fast-paced hospital setting. This full‑time role requires licensure and clinical experience in an acute care environment.

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Three (3) to five (5) years of inpatient experience, preferably in an acute, IRF, or LTACH setting.
• Current BLS certification required.

Responsibilities

• Coordinate care for designated patient population and reviews of admission records to ensure appropriate level of care.
• Identify discharge needs and coordinate discharge plans with the interdisciplinary team.
• Maintain and enhance payor relationships and support achievement of financial and quality outcomes.
