Job summary

Jobtailor is seeking an experienced registered nurse in New Jersey to develop and implement health service strategies and care plans. You will conduct home visits, assess medical needs, coordinate with care teams, and document activities in the electronic health record.

The role requires NJ licensure, at least 3 years of clinical experience, NJ Choice Certification, and willingness to travel up to 75% as needed. Mentoring and collaboration across interdisciplinary teams are essential.

Qualifications

• Active RN license in New Jersey
• Minimum 3 years of clinical experience in hospitals, home health, or ambulatory care
• NJ Choice Certification required for continued employment
• NJ Choice Certification exam score of 80% or higher
• Field mentoring as required by NJ Division of Aging Services
• Completion of state training modules
• Background in managed care preferred

Responsibilities

• Mentor new staff after achieving proficiency

• Conduct face-to-face home visits using comprehensive assessment tools for members enrolled in MLTSS and/or D-SNP/FIDE
• Perform in-person assessments for non-MLTSS members to determine medical needs and support appropriate service referrals
• Coordinate and collaborate with members, authorized representatives, primary care providers, and interdisciplinary care teams
• Schedule and participate in interdisciplinary meetings
• Advocate for members to ensure safe transitions and appropriate services
• Develop individualized care plans and authorize services within MLTSS/FIDE program benefits cost-effectively
• Document assessments, interactions, and care planning activities accurately and promptly in the electronic health record
• Apply critical-thinking and problem-solving skills in daily work
• Mentor new staff after achieving appropriate proficiency

Requirements

• Active, unrestricted RN license in the state of New Jersey
• Minimum of 3 years of clinical experience in settings such as hospitals, home health, or ambulatory care
• Residency in Middlesex County, NJ
• Reliable transportation
• Willingness and ability to travel up to 75% of the time
• Successful completion and maintenance of NJ Choice Certification, required for continued employment
• NJ Choice Certification exam score of 80% or higher
• Completion of state training modules
• Field mentoring, as required by the NJ Division of Aging Services
• Minimum of an associate degree in Nursing
• Experience in case management and/or discharge planning preferred
• Background in managed care preferred
• Crisis intervention skills preferred
