Jobtailor is seeking a registered nurse to develop and implement health service strategies and coordinate care for diverse members in Mercer County, NJ. You will conduct home visits, perform assessments, and collaborate with interdisciplinary teams to ensure high-quality, cost-effective services.
Responsibilities include documentation in the EHR, mentoring new staff, and traveling up to 75%. Requires NJ RN license, associate degree, and NJ Choice Certification, with preferred experience in case
Develop, implement, and support health service strategies, policies, and programs promoting high-quality care
Conduct face-to-face home visits using comprehensive assessment tools for MLTSS and/or D-SNP/FIDE members
Perform in-person assessments for non-MLTSS members and determine medical needs and appropriate service referrals
Coordinate and collaborate with members, authorized representatives, primary care providers, and interdisciplinary care teams
Schedule and participate in interdisciplinary meetings
Advocate for members to ensure safe transitions and appropriate services
Develop individualized care plans and authorize services within MLTSS/FIDE program benefits cost-effectively
Document assessments, interactions, and care planning activities in the electronic health record
Utilize critical-thinking and problem-solving skills
Mentor new staff after achieving appropriate proficiency
Demonstrates expertise in developing and implementing health service strategies and individualized care plans while effectively coordinating with interdisciplinary teams. Proficient in conducting assessments and advocating for members to ensure high-quality care and safe transitions.
