Job summary

Jobtailor is seeking a registered nurse to develop and implement health service strategies and coordinate care for diverse members in Mercer County, NJ. You will conduct home visits, perform assessments, and collaborate with interdisciplinary teams to ensure high-quality, cost-effective services.

• Residency in Mercer County, NJ.
• Reliable transportation required.
• Willingness to travel up to 75% of the time.
• Successful completion of NJ Choice Certification with exam score 80%+.
• Completion of state training modules.
• Field mentoring required by NJ Division of Aging Services.
• Minimum of an associate degree in Nursing.
• Experience in case management or discharge planning preferred.
• Background in managed care preferred.
• Crisis intervention skills preferred.

Requirements
