Qualifications

• NJ Choice Certification with minimum exam score of 80% and field mentoring component.

Responsibilities

• Conduct in-home assessments with members enrolled in MLTSS programs.
• Evaluate medical, behavioral, and social needs to support appropriate care and services.
• Collaborate with providers, caregivers, and interdisciplinary teams for coordinated care.
• Develop and manage individualized care plans to promote safety and independence.
• Participate in care team meetings and advocate for member needs.
• Document all assessments and care activities accurately in the EHR.
• Support members through care transitions and connect them with community resources.
• Mentor new team members as you grow in the role.

About This Role

The Registered Nurse Case Manager – Field in New Jersey is responsible for coordinating care for members with complex needs, ensuring they live safely and comfortably at home. This role is ideal for compassionate nurses who enjoy working independently in the community.

Highlights

• **Pay**: $66,575.00 - $142,576.00
• **Location**: Trenton, NJ
• **Schedule**: Full-time, Monday–Friday, 8:00 AM – 5:00 PM
• **Travel**: Up to 75% local travel required

What You’ll Do

• Conduct in-home assessments with members enrolled in Managed Long-Term Services and Supports (MLTSS) programs
• Evaluate medical, behavioral, and social needs to support appropriate care and services
• Collaborate with providers, caregivers, and interdisciplinary teams to ensure coordinated, holistic care
• Develop and manage individualized care plans that promote safety, independence, and quality of life
• Participate in care team meetings and advocate for member needs
• Document all assessments and care activities accurately in the electronic health record
• Support members through care transitions and help connect them with community resources
• Mentor new team members as you grow in the role

Requirements

**Education**:

• Associate’s degree in Nursing required, BSN preferred

**Experience**:

• At least 3 years of clinical experience in hospital, home health, ambulatory care, or similar settings

**Licenses & Certifications**:

• Active, unrestricted RN license in New Jersey required
