Job summary

The Dimock Center in Roxbury, MA is seeking a Case Manager for Acute Treatment Services to coordinate care for residents, provide advocacy, referrals, and aftercare planning. You will work with counselors and recovery specialists to support treatment goals and discharge planning, ensuring access to housing, employment, and resources.

Requirements include a Bachelor’s degree in a related field and at least one year in a substance use program; bilingual candidates are preferred.

Qualifications

• Bachelor's Degree in human services, psychology, sociology, health services, or related field.
• At least one year of experience working in a substance use program.
• Preferred: two years of experience with substance use and mental illness or homelessness.
• Bilingual in Spanish, Haitian Creole or Portuguese preferred.

Responsibilities

• Establish and maintain engagement with clients to meet treatment plan goals.
• Meet with clients weekly for care coordination and aftercare planning.
• Conduct assessments to identify needs, strengths and barriers.
• Coordinate discharge and aftercare planning well in advance of discharge.
• Maintain timely documentation and collaborate with treatment team.
