The position provides case management and housing advocacy to clients entering the system of care, assesses client needs, evaluates housing plans, and tracks progress toward housing goals. The Case Manager advocates for and supports clients enrolled in the Rapid Re-Housing and Homeless Prevention Programs.Essential Duties and Responsibilities:Program OversightAttend Family Support Team (FST), multi-disciplinary team (MDT), and other meetings as assignedProvide comprehensive case management services, including intakes, housing stabilization plans, goal setting, referrals, and ongoing monitoring.Use client-centered, harm reduction, Trauma-Informed Care, and Motivational Interviewing approaches to support housing stabilityMaintain accurate case notes using the GIRP (Goal, Intervention, Response, Plan) format and keep client files currentDevelop individualized case management plans, including financial and housing goals, with clients and familiesMeet regularly with clients to monitor progress, address barriers, and ensure program requirements are metProvide tenant education, advocacy, referrals, and connections to community resourcesEmpower clients by building on their strengths and addressing needs in a respectful, objective, and supportive mannerMaintain accurate and timely client information in HMIS and update program master logs dailyConduct home visits and Habitability Inspections as neededAssist clients with employment and housing-related barriersMaintain knowledge of community resources and connect clients to appropriate servicesFollow all agency case management policies and maintain an assigned caseloadParticipate in weekly Coordinated Entry System (CES) meetings, following all policies and procedures to request client matches and submit updatesAssist clients with housing searches, provide housing leads, and facilitate relationships between clients and landlordsSupport and mediate landlord/resident issues to help clients maintain housingComplete assistance recertifications, gather required documentation, and conduct client follow-ups as assignedUnderstand and prioritize quality outcomes for clientsTeam TasksCollaborate with staff, interns, volunteers, leasing agents, and other departments to support the agency’s mission and client housing goalsAttend and participate in staff, supervision, community, training, and monthly Housing Solutions team meetingsDiscuss client progress and needs, share resources, and coordinate services with the Housing Solutions teamFoster positive working relationships with case managers, leadership, and community partnersWork effectively in a fast-paced environment; multitask, adapt to changing workloads, and demonstrate reliability, trustworthiness, and empathyAdministrationMaintain complete and accurate client documentation, including stabilization plans, case notes, call logs, emergency plans, and related recordsSubmit and upload supporting documentation related to client services in a timely mannerAssist with program activities, data collection, and other administrative needsDevelop agency partnerships to enhance available servicesMaintain confidentiality and security of case files and client information and maintain knowledge of applicable HIPAA requirementsEnsure documents submitted on behalf of clients are accurate, valid, and timelyUtilize computers and appropriate software effectivelyRespond to calls and emails promptlyWork independently while effectively prioritizing tasks and deadlinesOccasionally lift items up to 25 lbs. and work for extended periods sitting at a desk and using a computerComplete other projects and duties as assigned by the Housing Solutions Supervisor and/or ManagerRequirements:Education and ExperienceBachelor’s degree in social work or a related field and/or four years of experience in social work or human services within a public or private organization requiredExperience working with individuals experiencing homelessness highly preferredGeneralValid California driver’s license, proof of insurance, and reliable vehicle required. Must be comfortable using a personal vehicle to conduct home visits, visit clients at shelters, meet clients in the community, and perform other job-related travelAbility to work effectively with diverse populations; demonstrate ethical behavior; plan, organize, and prioritize duties; provide crisis intervention as needed; communicate clearly and professionally, both verbally and in writing; maintain a positive, professional, and safe work environment; and establish effective working relationships with clients, landlords, social workers, and other service providersAbility to work in a fast-paced environment while maintaining confidentiality of sensitive client informationDemonstrated dependability, responsibility, and effective, respectful communication skillsProficiency in Microsoft Office and basic computer skills, including typing, email, and internet useAbility to work occasional weekends and/or evenings to respond to crisis situations as neededFluency in Arabic, Farsi, Spanish, or Vietnamese is highly valuedPhysical RequirementsAbility to lift up to 50 poundsAbility to sit for prolonged periods and work at a computer
