Job summary

Comunilife is seeking experienced Case Managers for the OMH Scatter Program in Queens. You will manage a caseload of about 22 clients, develop assessments and service plans, and conduct home visits and fieldwork to ensure comprehensive, client-centered care.

The role requires strong communication and documentation skills, with bilingual English/Spanish a plus. Benefits include health, dental, vision, dependent care, PTO, 401(k), and tuition assistance.

Qualifications

• Bachelor’s degree with two years’ case management experience
• Associates degree with four years’ case management experience
• High school diploma with six years’ case management experience
• CASAC-T or CASAC is a plus
• Knowledge of HRA Congregate Housing Program guidelines
• Direct service experience with people living with HIV/AIDS, substance use, serious mental illness, and homelessness
• Bilingual English/Spanish a plus
• 5 Day/35 hour work week Non-Remote Position (In-Office/Some Field Work)
• Excellent oral and written communication skills
• Proficient in Google Workspace and Microsoft Office Suite
• Familiar with AWARDS, CAIRS, CAPS

Responsibilities

• Complete comprehensive assessments, service plans, and progress notes
• Initiate and monitor clients’ needs for referrals and services
• Provide psychoeducation on Substance Abuse, Entitlements, etc.
• Deliver individual and group workshops
• Collaborate with interdisciplinary teams to ensure client-centered services and proper documentation
