Job summary

Comunilife, based in Bronx, NY, is seeking an experienced Case Manager to join its HRA Scatter Program. You will manage a caseload of about 22 clients, perform comprehensive assessments, and develop service plans with home visits and fieldwork.

The position emphasizes collaboration with interdisciplinary teams, client-centered services, and documentation. Candidates should have related degrees and/or CASAC status, bilingual English/Spanish a plus, and strong communication skills in a non-profit

Qualifications

• Bachelor's Degree in human services or related field with two years' case management experience.
• Associate's Degree in human services or related field with four years' case management experience.
• High School Diploma with six years' case management experience will be considered.
• Credentialed Alcohol and Substance Abuse Counseling (CASAC-T or CASAC) a plus.
• Knowledge of HRA Congregate Housing Program guidelines.
• Direct service experience with persons living with HIV/AIDS, substance abuse, serious mental illness, and homelessness.
• Bilingual English/Spanish a plus.

Responsibilities

• Initiate and continuously assess and monitor clients' needs for referrals and services
• Provide psychoeducation (Substance Abuse, Entitlements, etc.) to clients
• Provides individual and group workshops

Comunilife is looking for experienced Case Managers to fill open positions for their HRA Scatter Program in the Bronx.

The Case Manager's main responsibility is to provide case management services for a caseload of approximately 22 clients. The Case Manager prepares comprehensive assessments and service plans. Home visits and Fieldwork are required.
