Comunilife Inc. is seeking experienced Case Managers for the HRA Scatter Program in the Bronx. You will manage a caseload of about 22 clients, complete assessments, and develop service plans with home visits and fieldwork required.

Qualifications

• Bachelor’s or Associate degree with relevant case management experience.
• Knowledge of HRA Congregate Housing Program guidelines.
• Direct service experience with clients who are homeless, HIV/AIDS, or with substance use.
• Strong written and oral communication; proficient with Google Workspace and MS Office.

Comunilife is looking for experienced Case Managers to fill open positions for their HRA Scatter Program in the Bronx. The Case Manager’s main responsibility is to provide case management services for a caseload of approximately 22 clients. The Case Manager prepares comprehensive assessments and service plans. Home visits and Fieldwork are required.
