Qualifications

• Strong communication and interpersonal skills.
• Ability to manage multiple clients, appointments, referrals, and deadlines.
• Ability to work independently while remaining connected to a multidisciplinary team.
• Reliable transportation for community-based services.

Responsibilities

• Maintain and manage an assigned client caseload.
• Maintain regular contact with clients based on individual needs and service plans.
• Identify client needs related to behavioral health and resources.
• Connect clients with community programs and services.
• Complete referrals and follow up to ensure connection.
• Assist clients with appointments, applications, and benefits.
• Support clients in developing coping and daily living skills.
• Follow up after missed appointments or disengagement.
• Coordinate care with clinicians, nursing staff, families, and community partners.
• Document services, actions, progress, barriers, and next steps.
• Meet productivity and billable expectations.
