Job summary

Montefiore St. Luke’s Cornwall in Newburgh, NY is seeking a per diem Case Management Leader RN to screen patients for medical necessity at admission and coordinate interdisciplinary care across hospitalization.

The role requires NY RN licensure with Case Management certification eligibility within 18 months; BSN preferred, strong communication skills, and per diem shifts with an hourly rate of $61.18.

Qualifications

• Graduate of NLN Accredited School of Nursing, BSN preferred. Ability to read, write, and communicate in English.
• Minimum of one (1) year experience in an acute care hospital. Strong clinical, organizational, and interpersonal skills.
• Current New York State Licensure as a Registered Professional Nurse. Certification in Case Management required 18 months after eligibility. PRI certification preferred.

Responsibilities

• Screen all patients for medical necessity of admission using Level of Care Criteria.
• Coordinate interdisciplinary plan to meet patient treatment and discharge needs.
• Assess, plan, implement, monitor, and measure effectiveness of interventions to meet patient’s treatment and discharge needs.

Job Summary

Responsible for screening all patients for medical necessity of admission, regardless of point of entry, with the use of industry approved Level of Care Criteria. Provides the care and services of patients across the continuum of their hospitalization. The case manager assumes responsibility for an interdisciplinary process which assesses, plans, implements, monitors, and measures the effectiveness of interventions to meet patient’s treatment and discharge needs, which would achieve optimal clinical and resource outcomes.

Education/Training

Graduate of NLN Accredited School of Nursing, BSN preferred. Ability to read, write, and communicate in English.

Experience

Minimum of one (1) year experience in an acute care hospital. Strong clinical, organizational, and interpersonal skills.

Licensure
