Job summary

Montefiore St. Luke's Cornwall in Newburgh, NY is seeking a per diem Saturday Case Manager to coordinate care across the hospitalization continuum. Candidates must hold a BSN, MS in Social Work, English fluency, NY RN licensure, LCSW within 180 days, CCM certification within 1.5 years, and PRI certification preferred.

Job Summary

This is a PD Saturday Position Coordinates the care and services of patients across the continuum of their hospitalization. The case manager assumes responsibility for an interdisciplinary process which assesses, plans, implements, monitors, and measures the effectiveness of interventions to meet patient's treatment and discharge needs, which would achieve optimal clinical and resource outcomes.

Qualifications

• BSN Graduate of NLN Accredited School of Nursing Required
• MS Masters Degree from an accredited school of Social Work Required
• 1-3 years Experience in an acute care environment Required
• Ability to read, write, and communicate in English.
• Strong clinical, organizational, and interpersonal skills.
• Registered Nurse New York - New York State Board of Nursing Current New York State Licensure as a Registered Professional Nurse. Required
• Licensed Clinical Social Worker - New York State Board of Social Work within 180 Days Required
• CCM - Certified Case Manager - Commission for Case Manager Certification Certification in Case Management required within 1-1/2 Yrs Required
• PRI - Patient Review Instrument Cert - PHTC PRI certification Preferred
