Job summary

The Senate Residence in New York, NY seeks a dynamic case manager to support formerly homeless adults with severe mental illness, focusing on housing stability using a client-centered approach. You will work with medical, psychiatric, and community services to meet goals and ensure residency maintenance.

Position requires a Bachelor’s in social work or related field; Spanish speaking is a plus, and weekend/weekday coverage is possible. Salary is based on experience, with union benefits.”

Qualifications

• Bachelor’s Degree in social work or related field required.
• Experience with individuals with severe mental illness, substance abuse, or co-occurring disorders preferred.
• Spanish speaking preferred.

Responsibilities

• Provide case management services to a group of assigned clients; identify short and long-term treatment goals and help clients achieve them; coordinate with medical/psychiatric providers.
• Coordinate and follow up with onsite medical and psychiatric treatment providers; provide onsite medication monitoring; accompany clients to appointments.
