Job summary

Main Line Health in Malvern, PA is seeking an RN Case Manager or MSW Case Manager to oversee discharge planning for patients and families within inpatient and outpatient settings.

The role emphasizes coordination with payors, psychosocial support, and ensuring appropriate utilization of services. At least two years in hospital or rehab settings and CCM preference are noted; MSW or RN required. Day shift coverage with some weekend duties.

Experience

1. Minimum of two years recent experience in a hospital or rehabilitation facility required.
2. Minimum of two years recent insurance, case management experience preferred.

Education

Licensures/Certifications

CCM preferred.
