Job summary

Main Line Health is seeking an RN Case Manager or MSW Case Manager at Bryn Mawr Rehab Hospital in Pennsylvania. You will coordinate discharge planning, monitor patient outcomes, and engage payor representatives to ensure appropriate services.

Qualifications

• Minimum of two years recent hospital or rehabilitation facility experience.
• Minimum of two years recent insurance, case management experience preferred.
• MSW or RN required; CCM preferred.

Responsibilities

• Serve as patient program manager for discharge goals of the inpatient/outpatient team.
• Communicate with payor representatives to arrange services.
• Provide age-appropriate psychosocial interventions for patients and families.
• Monitor outcomes and utilization of services with the care team.
