Job summary

Main Line Health is seeking an RN or MSW Case Manager for Bryn Mawr Rehab Outpatient in Wynnewood. The role focuses on coordinating discharge plans, communicating with payors, and providing psychosocial support to patients and families. Eligible for tuition reimbursement up to $6,000 annually depending on status.

The position requires at least two years of hospital or rehab experience, MSW or RN, and CCM is preferred. Schedule is Monday–Friday, day shift, full-time with a competitive pay range.

Qualifications

• Minimum of two years recent hospital or rehabilitation facility experience.
• Minimum of two years insurance/case management experience preferred.
• MSW or RN required.
• CCM certification preferred.

Responsibilities
