Case Manager Pawtucket Health Home - $2500 Retention Bonus

This position is eligible for a $2500 retention bonus.

Job Summary: The Providence Center Case Manager Pawtucket Health Home will coordinate services for consumers to minimize stressors and facilitate the reduction of psychiatric and substance abuse disorder symptoms.

Encourage consumers to attain the highest possible levels of independence.

Responsibilities:
  • Coordinate a variety of services.
  • Treat consumers with respect.
  • Provide support counseling, problem solving, contracts, and limit setting.
  • Participate as a member of a multidisciplinary team in developing and implementing therapeutic services, including working with substance abusers, related to case management.
  • Make assessments and provide crisis interventions and risk management as needed.
  • Teach consumers adult daily living skills.
  • Integrate skills training into all aspects of Rehabilitation and Recovery Plans.
  • Initiate treatment and advocate for consumers within the network of care.
  • Negotiate with landlords, courts, families, the treatment team, and consumers as needed.
  • Transport consumers in own vehicle or Center van to activities and appointments.
  • Complete recordkeeping in accordance with HIS and other center requirements, maintaining confidentiality.
  • Maintain cooperative relationships with staff, consumers, community agencies, and the public.
  • Attend trainings, case presentations, and conferences.
  • Other duties as assigned.
Requirements:
  • Bachelor’s degree in Human Services or a related field from an accredited four-year college or university.
  • Experience in the Human Services field preferred.
  • Valid driver’s license and insured auto.
  • Flexibility.
