Case Manager ICT Pawtucket Health Home - $2500 Retention Bonus

This position is eligible for a $2500 retention bonus.

Job Summary:
The Providence Center is seeking a Case Manager ICT Pawtucket Health Home to coordinate services for consumers, aiming to minimize stressors and facilitate the reduction of psychiatric and substance abuse disorder symptoms. The role involves encouraging consumers to attain the highest possible levels of independence.

Duties and Responsibilities:
  • Coordinate a variety of services.
  • Treat consumers with respect.
  • Provide support counseling, problem solving, contracts, and limit setting.
  • Participate as a member of a multidisciplinary team in developing and implementing therapeutic services, including working with substance abusers in case management.
  • Make assessments, provide crisis interventions, and manage risks as needed.
  • Teach consumers adult daily living skills.
  • Integrate skills training into all aspects of Rehabilitation and Recovery Plans.
  • Initiate treatment and advocate for consumers within the network of care.
  • Negotiate with landlords, courts, families, the treatment team, and consumers on behalf of the consumer.
  • Transport consumers using own vehicle or Center van to activities and appointments.
  • Complete recordkeeping in accordance with Health Information Services (HIS) and other center requirements, maintaining confidentiality.
  • Maintain cooperative relationships with staff, consumers, community agencies, and the public.
  • Attend trainings, case presentations, and conferences.
  • Perform other duties as assigned.
Requirements:
  • Bachelor’s degree from an accredited four-year college or university in Human Services or a related field.
  • Experience in the Human Services field preferred.
  • Valid driver’s license and insured auto required.
  • Flexibility in work schedule.
