Job summary

Aurora Charter Oak Hospital is seeking a dedicated Case Manager for its Outpatient Department in Covina, CA. This full-time role involves coordinating case management, psychotherapy, and discharge planning to support patient and family needs.

Join our experienced multidisciplinary team and contribute to a patient-centered treatment program with a longstanding commitment to quality care for mental health and substance use disorders.

Qualifications

• Bachelor's degree in social work, psychology or social services.
• Master’s degree preferred.
• 3 years behavioral health experience preferred.
• Associate license such as ASW (Associate Clinical Social Worker), AMFT (Associate Marriage & Family Therapist), APCC (Associate Professional Clinical Counselor) preferred.
• Current BLS healthcare provider card required upon hire.
• Therapeutic Options training to be obtained prior to providing direct patient care without a preceptor.

Pay range: $23.00 - $29.00 per hour
