Charter Oak Hospital in Covina, CA seeks a Case Manager for the Inpatient Department to deliver quality case management, discharge planning, and psychotherapeutic services as part of an interdisciplinary team.

The role supports the hospital's patient-centered treatment program, with 8-hour shifts, full-time status, and a pay range of $23.00–$29.00 per hour. A BS in social work or related field is required, with Master’s preferred and current BLS certification.

Qualifications

• Bachelor's Degree in Social Work, Psychology or Social Services.
• Master’s Degree preferred.
• 3 years behavioral health experience preferred.
• Associate license such as ASW, AMFT, APCC preferred.
• Current BLS healthcare provider card required upon hire.
• Therapeutic Options training to be obtained prior to direct patient care.
