Aurora Charter Oak Hospital in Covina, CA is seeking a Case Manager for the Outpatient Department. This full-time position involves 8-hour shifts (AM), Tuesday–Saturday with a pay range of $23–$29 per hour. The role focuses on case management, psychotherapy, and discharge planning within a patient-centered behavioral health program.

Qualifications

• Bachelor's degree in Social Work, Psychology or Social Services required.
• Master’s Degree preferred.
• 3 years behavioral health experience preferred.
• Current BLS healthcare provider card required upon hire.
• Therapeutic Options training to be obtained prior to direct patient care.

Responsibilities

• Provide quality case management, psychotherapy services, and discharge planning to patients and families.
• Support the interdisciplinary treatment team to ensure integrated care.
• Contribute to individualized treatment plans aligned with patient needs and recovery goals.
• Participate in delivering evidence-based behavioral health treatments.
