Fortune Society is hiring an ATI Case Manager Mentor in Long Island City, NY. The position supports the Alternatives to Incarceration unit, handling intakes, caseload management, and mentoring to promote successful reentry and reduce recidivism.

Qualifications

• One year of case management experience preferred.
• High school diploma or GED/HSE preferred.
• Experience with mandated participants with criminal justice involvement.
• Bilingual English/Spanish a plus.
• Strong organizational and written/verbal communication skills.
• Lived experience with the criminal justice system strongly valued.

Responsibilities

• Conduct intakes for new clients enrolling into the program.
• Perform case management duties, including weekly individual sessions and referrals.
• Develop client schedules and treatment plans; monitor progress for court reporting.
• Provide internal & external referrals to address participants' needs.
• Enter data accurately into the agency database and support court reports.
• Mentor participants through sessions and facilitate groups when needed.
• Engage in outreach to maintain consistent participant engagement.
