Job summary

Saginaw Psychological Services, Inc. is seeking Adult Case Manager and Child Case Manager to join our Saginaw Main team. You will work under the Case Manager Supervisor to support individuals and families facing mental health and substance use challenges.

As a case manager, you will assess needs, develop action plans, connect clients with services, and monitor progress. The role requires compassion, organization, and strong communication across healthcare partners.

Qualifications

• Bachelor’s degree in social work or related field as listed in job specs.
• License not always required but preferred if applicable.
• Experience in case management within mental health or SUD settings is expected.

Responsibilities

• Reach out to clients assigned by supervisor and assess their needs.
• Develop action plans to meet client needs and secure resources.
• Provide psychosocial services in individual or group settings.
• Coordinate with external agencies to arrange support services.
• Maintain comprehensive client records and ensure confidentiality.
• Assist clients with applications for income, housing, and medical benefits.
• Coordinate treatment planning including aftercare and recovery supports.
• Encourage involvement in self-help groups and social support networks.
