Job summary
Loudoun County Government seeks a Mental Health and Substance Use Disorder Case Manager to provide targeted case management in the Coordination Services program, serving youth and adults with serious emotional disturbance and serious mental illness in home and community settings. The ideal candidate has a Bachelor's degree in a human services field and at least three years of related experience; bilingual in Spanish is a plus. Strong communication and teamwork are required.
Qualifications

Provides targeted case management services and links to needed services.
Responsibilities
Provide targeted case management to youth and adults with serious emotional disturbance and/or serious mental illness.
Coordinate with a multidisciplinary team to develop recovery-oriented plans.
Assess strengths and needs; monitor service delivery and progress.
Link clients to community resources and supports while promoting individual choice.
