Qualifications

Responsibilities

• Provide direct case management, coordination, linking and monitoring of services.
• Conduct and document assessments of needs for persons served.
• Maintain accurate records of persons served.
• Develop goals with persons served/families and other team members for appropriate plans.
• Implement and monitor Person-Centered Planning processes.
• Develop community connections and coordinate with schools, physicians, courts, and other programs.
• Comply with agency policies and confidentiality requirements.
