Job summary

Qualifications

• Bachelor’s degree in a related field is required.
• Must qualify as QMHP, QIDP, or CMHP according to program assignment.
• Experience with SMI and I/DD populations is preferred.
• Experience with an electronic health record system is preferred.

Responsibilities

• Provide direct case management services to persons served, including coordination, linking and monitoring of services.
• Conduct and document assessment of needs and develop goals with persons served and families.
• Maintain accurate and timely records and follow person-centered planning.
• Develop community connections and coordinate/link with schools, physicians, courts and other programs.
